We are interested in your experience and opinions on the care that your family and child have received to help improve the services we support. The information we gather will be used to help us improve the quality of services that your child and family receive. This survey will take about 15 minutes to complete.
Is there any risk to my privacy in taking this survey?
No. Your answers will remain completely confidential and anonymous. You will not be asked to provide any identifying information. No individual data will be reported. Your participation in this survey is completely voluntary. No matter what your experience, or whether you choose to participate, this survey will NOT impact your family’s or child’s eligibility for any services you receive.
